For New Patients
To expedite the registration process the first time you visit us, please feel free to download each of the following forms that pertain to you. Fill them out, and return them to us by e-mail, fax, or dropping off to either clinic. When you come for your first visit, please bring your driver’s license or state ID card, your insurance card, and your prescription for physical therapy from your physician.
Your appointment time will determine which therapist is available to treat you.
Please give us advance notice when you require reports for upcoming doctors’ visits.

Billing & Insurance
Patients are responsible for knowing the benefits their insurance company provides for physical therapy treatment. If a referral is required, please ensure that your primary care physician or referring physician obtains one prior to your first visit, in addition to a prescription.
We suggest that you confirm with your insurance company the details of your benefits. While we will work with you and your doctor, it is your responsibility to know your insurance companies rules and restrictions.
